We provide a downloadable Rx Form for ease of use.  For correct formatting the Rx form should be printed on two (2) Pages Per Sheet (8.5”x11”).

Rx Forms are also available through our “What do you need?” request form.

The Visible Metal Design Chart should be used for all Porcelain Fused to Metal (PFM) cases.  The chart should be referenced by the doctor and the technician to determine  the custom crown design.  Once a design is determined our Sallee Dental Studio’s team will ensure the custom visibility is implemented on your case and to your specifications.
